I agree that LIFE Christian Church may authorise on my child's behalf whatever medical treatment he/she may reasonably require. (This includes. but is not limited to. ambulance attendance and hospital treatment) I agree to pay all medical expenses incurred for the said treatment.
I understand that LIFE Christian Church will take all reasonable steps to provide a safe environment for my child and to ensure that all equipment supplied for a church activity and/or event is of fit for the purpose. I hereby agree to indemnify LIFE Christian Church including but not limited to its volunteers. employees and authorised officers. against any and all claims arising from. or in connection with. any loss or injury and/or damage that may be suffered by my child or that my child may cause to another person. and/or any loss or damage to property. equipment or personal effects belonging to my child or any other person. arising either directly or indirectly out of or in connection with the activity described in the schedule incorporated in this form.
Raise Youth services include photography and video recording. Images of people attending or participating in Raise Youth may be used in church publications. church buildings. website or social media channels. As a precaution your child's name will not be published or linked with photographs. by attending the activities of Raise Youth. you agree that we may use your image as mentioned above. If you have concerns please speak to our Raise Youth Team.
